| TRUSTS & POLICIES 12/29/10 The Accident Medical coverage is provided to Participating Organizations in the Youth Group Insurance Trust. A copy of the Trust will be provided upon request of a Participating Organization. General Liability Policies are available to enrolled Participating Organizations. A copy of the Policy will be provided upon request of a Participating Organization. When a Group applies for coverage, a Certificate of Insurance will be issued to the Group. Requests should be maid via email to the corresponding Home School Insurance Solutions Customer Care Division Representative with NCG Insurance. Refer to the appropriate representative per the first letter in your Group name.

School Legal Defense Association (HSLDA) does not assume any of the insurance
risk provided by the carriers. HSLDA is not involved in
the administration of these plans including premium collection or adjudication
of claims. |
